Nurse (Renal/Dialysis)

The RN is responsible and accountable for all elements of the nursing process when providing and/or directing patient care, procedural treatment, and healthcare delivery. He/She uses the nursing process to develop individualized plans of care and coordinates delivery of treatment for patients that require renal replacement therapy (e.g. hemodialysis, peritoneal dialysis, kidney transplantation, and/or palliative care advance care planning for end of life). He/she identifies patient's needs, sets Veteran-centric goals of care, and implements nursing cultural, and spiritual responses of patients and uses evidence-based care standards, knowledge, judgment, and skills based on scientific principles. He/she is ethical, responsible, and accountable for quality patient care and customer service. He/she uses evidence as the foundation for practice and assumes responsibility for lifelong learning and professional development. He/she is accountable for adhering to established policies, standards of care and standards of practice. He/she reports directly to the Nurse Manager of Renal/Dialysis Services and/or designee. The following roles and responsibilities include, but are not limited to:
Participates in performance measure improvement Participates in staff huddles, council and/or committee meetings Provides and/or delegates direct patient care within scope of authority Applies Standards of Nephrology Nursing Practice to all aspects of direct patient care and delivery of care:
Collects pertinent data/information relative to the patient's health and/or situation Analyzes assessment data to determine actual/potential diagnoses, problems, issues Identifies expected outcomes for individualized plans of care develops plans that prescribe strategies to attain expected, measurable outcomes Implements identified care plans through coordination of care and treatment delivery; employs health teaching and promotion to promote health and safe environments Evaluates progress toward attainment of goals of care and positive outcomes Provides leadership Veteran care coordination, healthcare delivery, and customer service Performs on-call duties for inpatient acute hemodialysis and peritoneal dialysis procedures Team leads, directs/delegates allied health care providers and support personnel with appropriate authority Strives to achieve certification in nephrology nursing Functions as a primary nurse for an ESRD patient panel in collaboration with the renal/dialysis interdisciplinary team of nephrologists, designated renal fellows, pharmacists, social workers, dietitians, and other adhoc clinicians. Functions as charge nurse during the absence of the Hemodialysis Care Coordinator, as delegated Functions as a renal/dialysis services nurse care coordinator, as delegated Hemodialysis (HD) Care and Vascular Access coordination Peritoneal Dialysis Care Coordination Kidney Transplant Care Coordination Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Experience:
Active RN License and BSN or MSN preferred 3 - 5 years experience performing hemodialysis procedures in the chronic setting 3 - 5 years experience performing acute hemodialysis procedures in hospital setting 3 - 5 years experience performing peritoneal dialysis procedures 3 - 5 years experience in kidney transplantation and chronic kidney disease care management Active CNN or CDN Active BLS Reference:

Heavy lifting (45 lbs and over); Heavy carrying (45 lbs and over); Straight pulling and pushing; reaching above shoulder; use of fingers; both hands required; walking and standing up to 8 hours; repeated bending; Ability for rapid mental and muscular coordination simultaneously; hearing aid permitted. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
