Vocational Nurse
The LVN/LPN at this level serves as the care coordinator for Telehealth Clinics including Telehealth Imaging, CVT, and VVC Clinic. The Telehealth Clinical LVN/LPN is assigned the duty of reviewing consults associated with the clinics, collaborating with clinic providers to coordinate pre-appointment testing, contacting Veterans to schedule telehealth appointments and providing patient education. The LVN/LPN supports PACT and Specialty Providers by completing clinical reminders and providing patient education appropriate to telehealth appointment such as instruction on using glucometers. Alerts providers of lab results and diagnostic tests for scheduled specialty CVT clinic appointments for further review. Responsible for the day-to-day operations of the Teleretinal, TeleDern, and CVT clinics, including patient education, and other duties assigned in the telehealth clinic. Serves as the site LVN/LPN imager for Telehealth store and forward applications, facilitator for real time Clinical Video Telehealth (CVT) encounters, including patient education activities, Primary Care as well as specialty care remote visits, and providing equipment for the Care Coordination Home Telehealth (CCHT) program. Demonstrates the ability to function in the independent role to appropriately prioritize overall patient care needs and adjust plans for care delivery. Knowledge and skill to promptly recognize potential urgent or emergent patient care situation, common walk-in in outpatient setting:
chest pain, breathing problems or other acute medical conditions. Performs Telehealth imaging, (scanning retinal images and skin lesions) including teleretinal screening, teledermatology Telewound care and other Telehealth programming as technology becomes available, as identified and referred by the primary care provider, other providers, and the imager. The Telehealth Clinical LPN/LVN will assist with the care management (works in partnership with assigned Primary Care Provider and panel of patients for effective disease and chronic care management), will satisfy the patients clinical reminders related to the imaging process and take necessary action based on the reader's recommendations. Assists in patient care under the remote supervision of an independent licensed healthcare provider. GS-7 Completion of at least one (1) year of additional qualifying experience at the GS-6 level or equivalent, fully meeting all of the performance requirements for the GS-6 LPN/LVN. Knowledge and skills necessary to carry out more specialized patient care duties, including assignments in selected, more complex inpatient and/or outpatient care settings. Knowledge base promotes the incorporation of current patient care data in actively contributing to the development of the patient's overall health plan and in carrying out assigned nursing interventions. Positions at this grade level require broader knowledge and skills because supervision is more general and employees are expected to be involved to a greater degree in the development and modification of treatment plans. Ability to assist RN and/or MD/DO to appropriately prioritize overall patient care needs and adjust plans for care delivery. Makes astute observations of subtle, less obvious changes, both physical and emotional, in patient's condition, and accurately and thoroughly documents care delivered and patient's response to care that could require immediate modification of the patient's care plan. At this grade level, the LPN/LVN is expected to participate to a greater degree in prioritizing, and, as appropriate, modifying patient care treatment. Ability to proactively assist the RN or MD/DO in addressing patient's needs for medication/treatments, accurately administering prescribed medications/treatments according to established policies/procedures and appropriately recording administration and patient's response to medications/treatments received.
